CASE MANAGER

Conducts comprehensive case management activities designed to support and facilitate client access to health and other appropriate services.

WHAT YOU WILL DO:

Conduct and completes intake and appropriate assessments.

Develop and implement appropriate client care plans. Review and revise care plan quarterly.

Make appropriate referrals for services as needed or indicated based upon client’s care plan. Follow-up on referral to assure completion of service.

Conduct home visits according to the Maternal and Child Health (MCH) service code.

Conduct appropriate Face to Face follow-ups according to the Maternal and Child Health (MCH) service code.

When identified, refer high risk clients to Quality Assurance Nurse for referral to High Risk Case Management program.

Provide clients with medical transportation assistance when appropriate.

Provide clients with Health Education materials during follow-ups or as appropriate.

Conduct proper case closure. According to the Maternal and Child Health (MCH) service code this should include, as appropriate for the client: PCP is in place and care is continuing; WIC is in progress; Immunizations are up to date; Medical insurance is in place.

Document all case management activities (home visits, follow-ups, referrals, services- immunizations, EPSDT visits, prenatal visits, transportation, etc.) utilizing the Cornerstone Information System in a timely manner. Maintain all other required documentation related to the case management process (consents, developmental screenings, etc).

Attends and participates in regular programmatic, departmental and agency staff meetings and trainings.

Other duties as assigned.

WHAT YOU WILL NEED:

*Associates degree required.
*Minimum of 1 to 3 years experience in health, social or human service or community based setting.
*Must have strong organizational skills.
*Excellent oral and written communication skills required.
*Knowledge of basic computer operations required.

REQUIRED LICENSES, CERTIFICATES, REGISTRATIONS:
VALID DRIVERS LICENSE REQUIRED
