Community Health Worker II – Clinical

Sinai Urban Health Institute (SUHI) is hiring full-time, culturally competent Community Health Workers (CHWs) to work at clinic and emergency room locations on Chicago’s South Side. This role is part of the South Side Healthy Community Organization (SSHCO), which is comprised of 13 healthcare organizations including safety net hospitals, health systems and Federally Qualified Health Centers. The model will better connect existing health organizations, increase access to care, address some of the most challenging health issues, and ultimately, build health equity and ensure stronger, healthier communities across the South Side of Chicago.

Under the direction of the CHW supervisor, the CHW is responsible for providing and coordinating health education and disease management education, assisting individuals with navigating the health and social care systems, and providing case management services (under supervision). The CHW works within an interdisciplinary team and serves as a bridge between the patient and the medical system by building trusting relationships with community members served by the health center or program(s).  Home visits may be required for some interventions.  The position is grant funded and salary range is $20-23/hour, based on experience. The role is non-remote; onsite attendance during the day shift is required Monday-Friday.
