Manages day to day operations of the Care Continuum department, oversight of RN case managers, social workers and case manager assistants. Covers other areas when director is unavailable. Overall responsibility for communication and coordination of care through the continuum, which includes discharge planning, coordination and executing plans for a safe discharge. Participates in multidisciplinary rounds and develops staff through education and training to effectively manage in their roles. Responsible for the progression of care, utilization of resources and community engagement.

REPORTING RELATIONSHIPS:   System Director of Case Management 

General Summary:

Manages and coordinates Case Management departmental operation activities.

Demonstrates effective problem solving, conflict resolution, negotiating skills and demonstrates effective time management. 

Ensures departmental and programs compliance with hospital and other governing agencies as well as hospital policies, standards and procedures (e.g. JCAHO, CARF, Public Aid). 

Participates on or in pertinent teams, committees, and groups representing program and/or hospital. Communicatesinformation to and from departments and implements processes to improvequality of patient care and admission process. 

Participates/Facilitates in unit based team huddles regarding patient’s plan of care 

Keeps abreast of professional issues through professional affiliations and education. 

Conducts self in a professional manner according to hospital and departmental guidelines.

Manages Case Management staff as appropriate, Including participation with hiring, evaluation, and discipline and ongoing caregiver direction and goal setting 

Covers a small case management caseload as necessary.

Consults with physician and team regarding patient or clinical situation and assists staff with execution of patient care plan. Maintains effective communication with physicians and other professionals.

Fosters an environment that is conducive to team work and encourages communication and motivates team members to make positive change.

Performs all other duties as assigned.


JOB REQUIREMENTS:

MINIMUM EDUCATION:
Current state of IL RN license
Preferred:  Master Degree in above fields
 
MINIMUM WORK EXPERIENCE:
Five  years experienceas a Case Manager/Social worker and at least two year’s supervisory experience.

 

REQUIRED LICENSES, CERTIFICATES, REGISTRATIONS: current state of IL RN license required.
Current license to practice in their field or equivalent
